
Minnesota DHS Aging and Adult Services Rate Study
The Minnesota Department of Human Services (DHS) Aging and Adult Services Division is conducting a survey of provider costs and wages to support the evaluation of the legislatively-mandated rate methodology for a number of services provided under the Elderly Waiver, Community Access for Disability Inclusion, Brain Injury Waiver and the Alternative Care and Essential Community Supports programs. DHS is required to provide a report to the legislature summarizing the results of its evaluation and providing any recommendations for changes to methodology (Minnesota Statute 256S.21, Subd. 2-3). Guidehouse Inc., DHS’s contractor for this project, is issuing the survey.
Completion of this survey is your opportunity to help inform the components and factors of the rate methodology for these critical services. DHS welcomes and strongly encourages all providers to participate in the survey process. Providers that do not have a business tax identification number, or use their social security number as a business tax identification number, do not need to complete a survey.

The cost and wage survey, and an associated instruction guide, is available for download from this project website for your convenience. There is one survey for all services listed above. Surveys are due Friday, March 21st, 2025.
