Minnesota DHS Aging and Adult Services Rate Study

The Minnesota Department of Human Services (DHS) Aging and Adult Services Division is conducting a survey of provider costs and wages to support the evaluation of the legislatively-mandated rate methodology for a number of services provided under the Elderly Waiver, Community Access for Disability Inclusion, Brain Injury Waiver and the Alternative Care and Essential Community Supports programs. DHS is required to provide a report to the legislature summarizing the results of its evaluation and providing any recommendations for changes to methodology (Minnesota Statute 256S.21, Subd. 2-3).

This is the second phase of a rate evaluation that began in January 2025. Guidehouse Inc., DHS’s contractor for this project, is issuing the survey. The web-based survey is available through the link above. Sign in using a Guidehouse Okta account. You will need to sign up for a Guidehouse Okta account if you don’t have one. Your survey is tied to that account, and selecting Back, Next, or Save for later saves your draft so you can return with the same account.

Completion of this survey is your opportunity to help inform the components and factors of the rate methodology for these critical services. DHS welcomes and strongly encourages all providers to participate in the survey process. Providers that do not have a business tax identification number, or use their social security number as a business tax identification number, do not need to complete a survey.

    • Customized Living Services
  • For Brain Injury Waiver and Community Access for Disability Inclusion:
    • Customized Living Services

The cost and wage survey is available through the web-based form at the link above. Instructions are incorporated in the survey form itself, and Guidehouse will make frequently asked questions (FAQs) available for download from this project website for your convenience. There is one survey for all services listed above. Surveys are due Friday, December 18, 2026.

Note for providers who completed a survey in 2025: Providers of these services may have completed an earlier version of this survey in Microsoft Excel during the first phase of the rate evaluation, when a similar survey was offered in January 2025. If your organization completed a survey in that time period, you may choose whether to:

  1. Request a copy of your previously submitted survey from Guidehouse at MNDHSRateStudy@Guidehouse.com, and make direct edits to that file with updated cost and wage information.
  2. Complete a new “blank” version of the survey using the web link.
